Dr. Furman does not participate or have contractual agreements  with any insurance plans.  This is sometimes is referred to as an "out -of- network" or " non-participant" provider.

Medicare refers to this as "opting-out" agreement.  The insurance claim will not be reimbursed  by Medicare

Payment is due at the time of service and patients are supplied with the appropriate forms to send to their insurance company for reimbursement.
